Customized Name Decals

Generally, these are sold in sets of two (one for each side of the tube). Names should be no more than 25 characters (special arrangements can be made for longer names) and is not limited to a person's name, it can be whatever you like: your bike club, sponsor (if you're lucky enough to have one!), nickname, business name, etc. However, I reserve the right to refuse any order containing vulgarities.

You can place an order in any one of the following ways:
1) Download the PDF (96k) by clicking here, print it, fill it out and mail it to me at the address below
2) Download the ePDF (96k) by clicking here, fill it out and email it to suzieq@suzieillustrator.com
3) Send me an email with a special order request (longer name, different color or size, etc.). Special requests may incur addition costs, but not necessarily. Be sure to include all pertinent information for shipping, etc.

Payment:
Payment options are listed on the order form. I accept personal checks, cashier's checks and most major credit cards (through PayPal). In the case of a personal check, please note that I will not ship an order until it has cleared through the bank.
